At Natural Health Direct Primary Care, LLC patients will be required to sign certain forms for DPC membership that acknowledge that we work outside of an insurance model, including a CMS opt out form, if applicable. Members of Natural Health Direct Primary Care will also become a part of our private member association as an agreement that allows us to collaborate and decide the care that is best for you. Our electronic medical record system and telemedicine platforms are HIPAA compliant. We strive to offer complete medical privacy and confidentiality.
